Its stunning Grand Feu cloisonné enamel dial features a nighttime depiction of central Milan, with city names presented in Italian, highlighting Milan in a striking red. The hour hands crafted from white gold showcase a skeletonized Southern Cross design, while the lozenge-shaped minute hand ensures clarity against the intricate enamel artwork. A 24-hour ring encircles the dial, distinguishing between day and night across 24 different time zones.

Encased in 18K white gold, the watch has a generous size of 40.2 mm in diameter and a thickness of 11.49 mm, complemented by distinctive skeletonized lugs. The caseband and repeater slide carry an elegant hand-guilloched hobnail pattern. Although protected from dust and humidity, it is not designed to be water-resistant. Two interchangeable case backs—one solid and one made of sapphire crystal—bear the inscription "Patek Philippe Milano 2026." At the heart of this masterpiece is the self-winding R 27 HU calibre, which uniquely combines World Time functionality with minute repeater capabilities through two classic gongs directly affixed to the caseband. This innovative movement, with a diameter of 32 mm and a thickness of 8.5 mm, boasts a power reserve of 43 to 48 hours. The mechanism is wound by a beautifully guilloched 22K-gold off-centre mini-rotor, featuring a Gyromax balance system with a Spiromax balance spring. Completing the look is a luxurious dark-blue calfskin strap, hand-stitched and grained, fastened by a white-gold fold-over clasp.
